Reissuing a user ID and a password
If you have forgotten your user ID and password, you can get your user ID and password reissued with the following form. Please note the following points prior to use.

1. The reissued password is a temporary password for security.
After reissue, log in [Change Registered Info] with the user ID and the temporary password, and be sure to change the password.
(*The user ID cannot be changed.)
2. The reissued user ID and the temporary password will be sent to you at your registered e-mail address.
If you have registered a wrong mail address or have changed mail addresses, you will not receive the confirmation mail. In this case, please contact us at info@fa-plaza.com
